Hand Rejuvenation with Injectable Fillers

Botox and filler injections for facial lines and creases have become more mainstream, creating refreshed and more youthful appearing faces. When done in combination with facial, breast, and body rejuvenation surgery, we as plastic surgeons have been able to “turn back the hands of time” for many people. Reduce Noticeable Wrinkles

One of the best ways to look younger is to address fine lines and wrinkles on the face. Depending on the specific area of concern, our team recommends Botox or soft tissue fillers.

Botox is generally injected between the eyebrows and on the forehead into the muscles that contract and form wrinkles. The product paralyzes these muscles, causing the overlying wrinkle to soften. Soft tissue fillers like Restylane and Perlane are typically used for deeper wrinkles and folds, such as the nasolabial folds. When injected, these substances act like a cushion to fill in wrinkles.

Botox and dermal fillers are both performed during a quick, in-office procedure. Treatment is generally well tolerated and should be repeated every few months to maintain the effects.

Avoiding Smiling to Prevent Wrinkles? Luckily, You Don’t Have To

Preventing wrinkles before they appearLast month, Tess Christian, a 50-year-old cooking instructor from the U.K., made headlines when she admitted she hasn’t smiled in the past four decades. She has nothing to be sad about; actually, her perpetually neutral expression is her effort to maintain a youthful appearance.

“I don’t have wrinkles because I have trained myself to control my facial muscles,” Christian told the Daily Mail. “Everyone asks if I’ve had Botox, but I haven’t, and I know that it’s thanks to the fact I haven’t laughed or smiled since I was a teenager. My dedication has paid off; I don’t have a single line on my face.”

While Christian’s strategy may seem extreme, it’s true that whenever we make a facial expression, such as smiling, talking or frowning, a fold or crease appears on the skin. Over time, these lines become permanently etched in the skin. Because of these dynamics at play, Christian’s strategy isn’t necessarily misguided.

Admittedly, the Plastic Surgery Group finds it a particularly unpleasant way to prevent wrinkles. Smiling is a spontaneous behavior that communicates our joy to others. However, we understand the desire to have smooth and wrinkle-free skin. We have some suggestions for preventing facial wrinkles or treating existing lines. We think you will find them more reasonable than censoring your natural, beautiful smile.


Tess Christian made mention of Botox in her comments, saying that many people assume she has regular treatments with the cosmetic injectable to maintain smooth skin. We have to agree that Botox is one of the (if not the) most effective wrinkle reducers used today. When injected into specific areas of the face and neck, it relaxes the facial muscles that cause lines and wrinkles. Established lines will smooth out and new wrinkles will not form. Botox injections not affect the muscles that you need to smile, eat, open your eyes and make other normal facial expressions.

We do want to clarify something that Christian said about Botox, which is that people ask her if she has had the treatment. When it is administered properly, Botox is undetectable. No one will ask you whether you’ve had Botox injections. The results are subtle and natural looking.

Injectable Dermal Fillers

Like Botox, dermal fillers are cosmetic injectable products; however, they function in a completely different way. When injected under the skin, products like Restylane, Sculptra and Perlane act as an inflatable cushion to fill in facial lines and wrinkles and support the structures of the face.

Laser Skin Resurfacing

Laser skin resurfacing is another way to improve the appearance of wrinkles. Treatment involves removing the outer layer of wrinkled, damaged skin to reveal fresh, healthy skin underneath. Microdermabrasion and chemical peels accomplish the same objective, but with different technology.


If you are concerned about deep wrinkles coupled with other signs of facial aging (e.g., sagging, skin, fat displacement and loss of muscle tone), comprehensive correction in the form of facelift may be your best option.

During facelift, the underlying structures of the face are lifted, fat is redistributed and muscle tone is restored. The skin is re-draped more tightly over the facial structures, which smoothes out wrinkles and facial folds.

“Which would be better for my face/neck laxity: Venus Freeze or Ulthera?”

After a consultation with one of our doctors who determines you are a good candidate for one of our non-surgical procedures like Venus Freeze or Ulthera, how do you decide which procedure is right for you? Nancy Dugan, our technician specializing in the energy based procedures suggests the following:

Patients who are deemed good candidates for non-surgical procedures may consider combining the effects of Venus Freeze and Ulthera. Though they work in different ways, they are in fact complimentary procedures. Each works in different, but partially overlapping skin layers. The Ulthera is a single procedure that works on 3 depths: 4.5 mm, 3.0mm and 1.5 mm into the skin. It bypasses the surface and does its work underneath; tightening the skin, building collagen, lifting and improving over a period of up to 6 months after the treatment. It can be done for the brow, temples, under eyes, cheeks, upper neck, lower neck and décolleté. It is non-ablative, and works beneath the surface, so there is little to no outside evidence of the treatment, and no down time. Patients usually repeat the Ulthera yearly, though there are some who want more improvement that do it 2 or 3x per year. There are ways to customize the Ulthera treatment for patients with thicker or thinner skin, and techniques to address different patterns of laxity and wrinkling.

Venus Freeze works via radio frequency and magnetic pulses in a non-ablative fashion as well, though not as deep, nor is it as invasive as the Ulthera treatment. It also tightens the skin and builds collagen, though it requires weekly (6-10) treatments to do so, then we usually do a monthly touch up with the regular facial treatment. It’s a terrific way to enhance and speed up the effects of Ulthera, so we consider them to be complimentary treatments that can be used together or separately. Venus Freeze treatments can be started anytime after 2 weeks following an Ulthera procedure. It can be used on the face, neck, décolleté, arms, thighs, buttocks, back, stomach & flanks/hips. The body treatment with Venus Freeze tightens the skin, shrinks fat cells, and reduces the appearance of cellulite.

Just for comparison, because the subject often comes up, laser/light treatments work in the upper area of the skin to “ablate” the surface, after which the skin will heal in a more youthful pattern. Laser treatments can go as far as 1.5mm deep, which may require healing time, and create peeling, redness etc. Lasers are superior at improving the surface texture, capillaries, and brown spots. Ulthera and Venus Freeze are not lasers. They are skin tighteners that work under the skin, and don’t ablate the surface like a laser, which is why they don’t require downtime.

Non-Surgical Facial Rejuvenation Rising in Popularity

Non-Surgical Facial Cosmetic ProceduresAccording to statistics released by the American Society of Plastic Surgeons, minimally invasive cosmetic procedures continue to outpace surgical procedures at a tremendous clip. ASPS reported 6.3 million Botox treatments in 2013 compared to just 133,000 facelift procedures.

What’s interesting about ASPS’s data is that the number of facelift procedures hasn’t changed significantly since 2000. However, the numbers for Botox have exploded. In 2000, only 786,911 treatments were delivered, which translates to a 700 percent increase between 2000 and 2013. (To be fair, Botox wasn’t FDA approved in 2000; it received safety clearance two years later.)

Pharmaceutical companies in the beauty industry have struck a chord with individuals who want to age gracefully. Every year, these companies release new minimally invasive products to meet the voracious demand of their consumers.

Let’s explore some of the reasons non-surgical facial rejuvenation treatments like Botox and dermal fillers are becoming so popular:
